Health, Social Care & Well being Strategies are the key local delivery mechanisms for bringing about sustainable health improvement in our communities. The Health Improvement Team has assisted in and continues to support the development and effective implementation of these strategies at a local level.
Health, Social Care and Wellbeing Strategies
The local Health, Social Care & Well-Being Strategies - covering each of the 22 unified local authorities in Wales - are drawn together by the local HSCWB Strategic Partnerships which consist of representatives of local stakeholders, including the statutory, voluntary, community and private sectors. The strategies cover a range of activities; from preventative action to improve health and reduce the risk of ill health, through to care services provided by each Council, the National Health Service, and the voluntary and private sector agencies. The strategies are all based on local needs assessments that have been carried out by the NPHS. The HSCWB strategies endeavour to provide local partnership solutions to the local priorities identified, but in a national policy and legislative context.
The first Health, Social care and Wellbeing Strategies operated between the 1 April 2005 and 31 March 2008. The second round of HSCWB strategies have now been developed and will operate from the beginning of April 2008 to the end of March 2011. The second round of strategies show a mature approach to identified area priorities, with coordinated and fully endorsed action plans agreed across the Partnership. Many strategies highlight the need for more emphasis on sustainable preventative programmes in terms of gains and improvements to individual, family and community health and wellbeing.
